Scheduling a Vision Screening

The Calendar below is the scheduling vehicle for Lions District 20E2 PlusOptix Vision Screeners. To schedule the camera, follow the process documented below.

On the first of each month there is an entry for EACH of the calendars in our District.  If you open this entry, it will explain who to contact to reserve the screener for a screening event.  To make the reservation, you must email or call the contact and request a particular date.  We should avoid reserving a screener for an extended length of time since we want the screeners fully utalized.  I would recommend a couple of days before and after each screening as a guideline.  If you have several screenings scheduled, either make them as close together as possible or release the screener between events, making it available for others to use.

Vision Screening Followup

After EVERY screening event, whether it is the first of a series of events or the only one you are doing,  Select This Link and a form will appear.  Fill in the blanks and your reporting requirements for the screening are completed.  Once you select the 'Submit' button at the bottom of the form, you're done!  The information you entered will go into a spreadsheet available to a select group of people who will use it to track the number of screenings, number of children screened, number referred, etc.  If you have problems while running a screening or with the screening device, please describe the issue in the comments area of the form.

THIS REPORT MUST BE MADE ONLINE WITH A CONNECTION TO THE INTERNET.  IT CAN BE MADE FROM ANY COMPUTER WHETHER IT IS YOUR PERSONAL COMPUTER, A PUBLIC COMPUTER (LIKE THE LIBRARY) OR A COMPUTER WITH INTERNET ACCESS AT THE SITE WHERE THE SCREENING EVENT TOOK PLACE.

NOTE:  Make a single entry for each event.  Before being submitted, you can make any needed changes to the form.  After you submit the form no additional changes can be made, if there are changes necessary, you must submit a complete new form and notify your screener contact of the duplicate entry.
